Free Health Screening

Thank you for your interest in our Free Health Screening.

This is as easy as 1 - 2 - 3.

1.  Download the PDF file and email to info@4absolutehealth.com or fax a

printed copy to 503-630-2002.

2.  Watch your email for your personal invitation to take the test. This

email will include a link that will allow you start your test. This test is

comprehensive to give yourself a baseline that can measure your

progress as you start this journey to better health. If you need to stop

in the middle, it will be automatically saved. Sign in with your

username and password at a later time.

3.  After finishing your test, wait for your results by email. This will

include an easy to read bar graph, your top 3 health issues, and

suggested products to address these issues. These are suggestions
